We believe that everyone has the right to participate in the everyday activities of life that most adults in our society take for granted, like shopping and learning money management skills, using local parks for recreation, and volunteering opportunities.

Supported Community Connections

People with developmental disabilities enjoy the same rights and responsibilities as all other people. They should be allowed to be active members of their mainstream community and experience all aspects of community life like every other adult. In return, communities are healthier when all people contribute according to their strengths.

We believe that everyone has the right to participate in the everyday activities of life that most adults in our society take for granted---like shopping and learning money management skills, using local parks for recreation, and volunteering opportunities. Many people with developmental disabilities do not have the skills to participate in the community unassisted. Even when they are physically integrated in the community, they often require supports to assist them to take part in those activities that the rest of us take for granted. The support a person receives can have a fundamental impact on his/her quality of life.

Day Program Highlight

Supported Community Connection activities in which they can participate to improve their quality of life include:
– Entertainment/Cultural Activities
– Attending clubs or organizations
– Going to a shopping mall to learn money management skills
– Visiting museums and libraries
– Bowling for recreation and learning math skills
– Going to concerts, picnics, parks, zoos, etc.
– Recreation for fun and for exercise
– Walking, jogging, hiking, swimming
– Participating in sports

Day Program Details

Stellar Care’s Supported Community Connections Program offers the support persons with developmental disabilities need to enable the increased mobility, decision making and self-advocacy, money management and consumer skills, relationship building, and communication skills that allow them to live, thrive and take their place in the world.

For people with significant support needs, it is critical to have support staff who are competent at providing personal care. Stellar Care hires the best employees we can, and we take pride to find good fits between our direct support staff and the person served. Stellar Care staff are trustworthy, person-centered, supportive, creative, reliable, positive people who are respectful of the persons they serve.

The person served is given the opportunity for recreation, socialization, communication, skill building, and physical fitness as outlined in their Individual Service Support Plan (ISSP). Services will be provided based on the person-in-services’ level of funding. Staff ratios are 1:4 at maximum.

Door-to-door transportation: Stellar Care staff will pick up at the person’s home and transport them to activities in their local community.
